This position exists to direct and manage staff, services and outcomes for DHS's federally funded Money Follows the Person (MFP) Rebalancing Demonstration Grant, Money Follows the Person Tribal Initiative (MFP TI), and Money Follows the Person Capacity Building Grant. Minnesota's MFP rebalancing demonstration grants are known as "Moving Home Minnesota" and "Moving Home Minnesota Tribal Initiative." The MFP and MFP TI projects are systems transformation grants designed to "rebalance" Minnesota's Medicaid system and services.
Moving Home Minnesota (MHM) begins as a Medicaid benefit available to individuals who wish to move from institutional to community-based homes. MHM is a dynamic and agile benefit set designed to complement Minnesota's existing state and waiver plan for the Home and Community-Based Services (HCBS). MHM is the testing ground for the development of demonstration HCBS that the state may want to later add into state and waiver plans, as well as developing supplemental services that may not be otherwise allowable as a Medicaid benefit. These activities receive an enhanced federal match to support the state's rebalancing and systems transformation activities.
As rebalancing and systems transformation grants, this position is heavily focused on strategic leadership and relationship-building, laying the groundwork for a new collaborative leadership structure across multiple administrations to ensure the department meets the MFP, MFP TI, and state goals to transform our long-term services and supports system. This includes management of the MHM Rebalancing Team and initiatives, including the MFP TI (and DSD) Tribal Coordinators and Capacity Building grants, data and research, and team members that support innovation and systems transformation across the department. It will also include coordination of rebalancing initiatives across the department and with the MHM Transition Team.
